Dude, of course you won't get sick eating this way. Eating fresh fish out of the sea is the BEST and SAFEST way to enjoy raw. In Japan, this is what people do everyday when they live near the sea. Nothing new.

Actually it was difficult. Mostly because of the multitudes of mosquitoes and no-see-um bugs that bit me. I nearly went insane because my entire body would itch like crazy and there was no relief.

The act of eating it would kill it. If you are referring to a small fish, like an anchovy or sardine, no, I wouldn't eat the whole thing alive. The bones would present a problem. If I were to bite into a living fish it would flop around and be difficult to eat. I have eaten live crayfish before. As I caught them in a river I just broke off the tail and ate it while the crayfish was still alive.
