It's very rare for a surfer (or anyone, for that matter) to be attacked by a shark. Not a cheap thrill, either. Takes a lot of time/practice/training to ride big waves.

It's actually fairly standard. At a certain point (different for all surfers) the leash simply becomes a liability and incredibly dangerous. Ropes and moving water are one of the most deadly combinations on Earth.

Note: It helps when you have a crew of jet skies to recover your board when you bail off of it and sponsors to buy you new boards when they snap in huge surf.

Sorry that I am gonna be negative here but I find this top 5 lost utterly pathetic. I watch surf video's almost daily and have seen wipeouts on Jaws and Mavericks 10x worse than your top 5. Watch the Mavericks video on Feb 4th and you will see 3 minutes of wipeouts worse than these.

The first two WERE Jaws bud. The 3rd and 5th were at Nazare which all the big wave guys acknowledge as pretty much as gnarly as anywhere. The 4th was Shipsterns which is one of the heaviest slabs in the world. You clearly have no idea what you are talking about.
